Conoid teeth, a form of isolated microdontia, primarily affect the upper lateral incisors, characterized by a conical crown, reduced mesiodistal diameter, and converging proximal surfaces, compromising smile aesthetics due to diastemas and disharmonious proportions. This literature review assesses the role of Orthodontics in the aesthetic management of this condition, emphasizing digital technologies such as clear aligners, ClinCheck, and Invisalign Smile Architect. Orthodontics provides a conservative approach, correcting dental positions and inclinations, often complemented by composite resin restorations or ceramic veneers. Digital planning tools, including Digital Smile Design and Smile Architect, optimize tooth movement and restorative design, minimizing invasive preparations and enhancing aesthetic predictability. Integration with Restorative Dentistry and Periodontics is crucial for harmonious outcomes, considering dental proportions, gingival harmony, and patient satisfaction. The scarcity of robust clinical trials underscores the need for further research to validate interdisciplinary approaches and evaluate the efficacy of digital tools.
